Abstract

Le reflux du liquide gastrique, acide dans la majorité des cas, se traduit par une baisse du pH œsophagien. La pHmétrie permet d'apprécier le coefficient d'acidité (pH inférieur à 4), rarement alcalin (pH supérieur à 7) et posant alors des problèmes d'interprétation de diagnostic difficiles à résoudre. Le développement rapide de cette technique en pédiatrie tient:—aux progrès réalisés dans la miniaturisation des sondes et à l'apparition d'une technologie moderne selon le principe du Holter, permettant le recueil facile du signal sur traitement informatique;—à la reconnaissance de formes atypiques du reflux gastro-œsophagien (RGO): respiratoires, laryngées, rhinolaryngées, neurologiques et sa possible implication dans les syndromes de mort subite du nourrisson;—à la nécessité d'apprécier de façon la plus objective l'efficacité des différentes thérapeutiques médicales ou chirurgicales.
